These names are the vocabulary used by release-week guides, not a KONAMI-issued bestiary or an exact damage table. “Motionless” and “Fallen” can refer to the same ambusher in fan descriptions. Scene silhouettes and ending imagery should not be counted as extra ordinary enemies without an encounter to explain.

When do the first monsters appear?

The first hostile gameplay section is Chapter 2, Escape, after the opening phone sequence takes Simon into the Otherworld and the CRTV is introduced. The recording’s 3:08 — Escape chapter ▶ is a useful starting checkpoint; it is not a universal three-minute encounter timer.

Early enemies do not require you to have the late-game gun inventory. Learn to read the CRTV, use cover and continue toward the door/objective. The CRTV tracking guide explains why a detected signal is different from being spotted.

What the October 8 patch changes

  • Story and Normal: The Weight’s grab now causes heavy damage instead of automatic instant death. Its hunting/searching behavior also changes.
  • Ordinary enemies: successful melee attacks produce fewer immediate counterattacks; actively blocking with a melee weapon prevents their grapple attempts. The Weight is specifically excluded.
  • Enemy reactions to running, firearm noise and entering hiding places are reduced. This does not establish that any of those actions is silent or always safe.

The relevant official release is patch 1.4.0, shown in the menu as v1.5.154194. An older guide saying every grab kills you, or that a Sorrowful grab always bypasses an active melee block, can describe a different build.

Widows and the final-boss boundary

Missing The Point Entirely requires silencing 30 Widows. Richard’s Part 5 CEG route supplies these enemies and the Shotgun; trophy progress continues across playthroughs. A claim that one recorded shot count always works ignores distance, aim, difficulty and damage already dealt.

The roaming Weight appears in the altered ward and SWDP routes. Its final battle is Eraser, Chapter 16. Use the pursuit guide for reaching controls, and the final-boss guide for the repeated damage-and-interaction objective.

What about nurses and unnamed monsters?

The recurring combat list checked for this guide does not identify a separate Nurse enemy class. Zoe’s nursing role and the hospital setting do not establish that the classic Bubble Head Nurse appears here. Match a screenshot or attack cue to the table rather than importing a monster from another Silent Hill game.

A “lung monster”, “invisible enemy” or “huge monster” search does not give enough detail to name a new species. For an obstructed route, the region, chapter and visible action are more useful than an guessed label. Story symbolism may explain a design, but it does not establish health, stun duration or an invulnerable phase.
